Israel's coronavirus exit strategy chaos exposes all of the government's ills - Bulletin Mail

Prime Minister Benjamin Netanyahu’s admirers around the world admire how Israel has coped with the coronavirus crisis. However, the low mortality of COVID-19 says nothing here about the complete anarchy of the exit strategy. It reached new heights last week with the start of the reopening of the education system.

After several twists and turns, the picture became clearer on Saturday night. Schools will reopen to 1st – 3rd grade and high school in the last two years – but not kindergartens as planned. Hared students return to school, but only 7th – 11th grade.

However, it turns out that the decision is only a recommendation. In some cities, classes continue on Sundays. Others, including Tel Aviv and Be’er Sheva, said they needed more time to prepare and would open schools later this week.

As throughout the crisis, the government has to make decisions in times of great uncertainty. Studies from different countries and a specific study by the Gertner Institute for Health Policy and Epidemiological Studies in Bnei Brak, a highly ultra-Orthodox city, did not clearly answer the key questions: To what extent are children at risk of infection? (It is known that children can become infected, although less than adults, but the frequency is still unknown.) To what extent can children, especially those who do not have the typical symptoms of COVID-19, infect adults?

At the same time, the coronavirus exposes all the major troubles of the Israeli government – belated and improvised planning, preparation and decision-making, along with surrender to pressure groups. The latter led to the reopening of higher classes, but not the lowest, in Hared schools on Friday, at the suggestion of Arye Dery, the interior minister and chairman of Shashi.

The differences reflect a deep conflict between the Ministries of Health and Education. The National Security Council report said that the former did not give the latter clear instructions on what the Ministry of Education needed to prepare to continue its studies.

To complement this unfortunate picture, Haim Rivlin, CEO of Hadassah Medical Center, Prof. Zeev Rotstein, followed on Thursday on Kanal 13’s TV show “Hamakor”. Rotstein runs two Jerusalem hospitals under the organization as a private company that ignores the health ministry. By letting the reporter record conversations with Prime Minister Benjamin Netanyahu, health ministry officials and Mossad department heads – probably mostly without their knowledge – rarely allowed viewers to peek behind the curtain on the common front, which was introduced in media briefings. It was frustrating: power struggles, knives drawn, mutual accusations, confusion and a little panic.

Coronavir party

After the end of Passover in mid-April, the disruption of the infectious party began, which began with the phasing out of restrictions. The streets and even some beaches and parks are full of people again. Social distancing is perceived as a recommendation. Masks? Sometimes.

Unnecessary restrictions on outdoor use within a radius of 500 meters from home have been lifted. So is the pretense that the police are enforcing the order (still in place) to stay within 100 meters of their home. Ultimately, it can be said that there are masses of people on the streets, some of them behaving like a coronavirus is a distant nightmare that no longer threatens us.

The results will be known in the next two weeks. The healthcare system predicts a certain increase in infection, probably quite modest at the moment – less than 200 new confirmed cases per day; on average, about 1.7 percent of the daily tests are positive. The coronavirus has so far hit Israel to a lesser extent than other western countries. If hot weather does limit the risk of infection, its increase may be relatively modest. If not, we could go back to the beginning and the Ministry of Health once again maintains strict quarantine. However, it has problems ensuring public cooperation in view of the loss of people’s faith in politics.

If there is no dramatic outbreak of the virus in the coming period, the economy will gradually return to normal, monitoring specific areas of spread. The gap between such communities and mainly ultra-Orthodox and the rest of the country is becoming clearer. The general assumption, which is only a conscious theory, is that a new outbreak is expected in the autumn, possibly with seasonal flu. Until then, the health care system has set a goal of having 7,000 fans in Israel. This is a huge number (currently less than 100 patients work on the fans), but there is also a need for qualified staff who know how to use them.

Unwanted

In the ongoing coalition talks between Likik and Kahol Lavan, the Ministry of Health is still a child, neither of whom wants custody. Yaakov Litzman resigned on the orders of his rabbi leader after the pandemic revealed the extent of the failure of his role. Gabi Ashkenazi, the co-leader of Kahol Lavan, was in no hurry to volunteer. Now another name has come: Expert, CEO of Sheba Medical Center in Tel Hashomer, Professor Yitshak Kreiss, whose candidacy to represent Kahol Lavan was recommended in this column at the end of March.

The reason why politicians are afraid of the health portfolio is clear. In this situation, it seems like a political abuse. The economic crisis, which is not expected to worsen until the coming months, will strengthen the decision-making role of the Ministry of Finance, certainly with the arrival of a new minister who will have to make budget cuts against the background of a growing deficit. The Ministry of Health is very dependent on the Treasury, even in normal times. The purchase of each new MRI machine requires the signature of the budget department of the Ministry of Finance. The participation of the Ministry of Finance will be tightened in the light of the crisis, making the Ministry of Health a kind of subcontractor of the good grace of the Treasury.

In addition, negotiations are expected to begin on a new salary agreement for doctors. A contract with doctors a decade ago highlighted major achievements that improved pay, especially for their older members. The lack of preparedness that emerged at the beginning of the coronavirus crisis is now focusing on more hospital beds and more ICU beds and fans, although so far the limited spread of the virus has not dramatically tested or brought the healthcare system closer. to the dreaded collapse.

The coronavirus is an exceptional and very rare event, an extreme first-time scenario. An argument will soon emerge: will the uncovered gaps call for improved preparedness at the usual time, or should resources be diverted based on the system’s ability to improvise, as has been the case throughout the current crisis?

Despite the billions that have been allocated to the Department of Health over the past few months, dilemmas remain. We still do not know the cost of the damage to human health that was not diagnosed or treated when the healthcare system was deeply in crisis with the coronavirus. It takes time and costs a lot to fill in the gaps. Funding for more hospital beds and doctors next year will also come from government-funded medicines, treatments and technologies. Budgetary problems are expected to persist and the Treasury will have to take into account other needs, such as helping the unemployed and small businesses. Potential ministers are aware of these problems and that is why the line of candidates is so short.
